The ACHIEVE Phase I Pilot study partnered with Piedmont Health Services and engaged with three of their outpatient clinics in rural counties in NC to test implementation strategies for the O-HTN bundle, and a coalition of clinicians, community members, and patients partnered with the ACHIEVE study team to advise and provide feedback on the feasibility of the strategies for the outpatient setting and to develop resources for patients and providers that are relevant and acceptable to the community.
