Implementation Teams will be formed at all participating clinics during the Implementation Phase. Implementation Team members are responsible for leading their clinics to integrate and sustain the components of the O-HTN bundle. They will participate in training,monthly facilitation, and simulations if they are also part of a Care Team (see definition below). There will be one Implementation Team per clinic. At a minimum, Implementation Team membership should include:
- A Provider Champion (physician, midwife, nurse practitioner)
- A Nurse
- Medical Support Personnel
- Administrative Support Personnel
