Guides created by ACHIEVE to support both patients and providers in their conversations about hypertension. These guides are developed in direct response to feedback received by community members through patient workshops and story circles conducted by ACHIEVE’s Community Engagement Core. Three types of guides are created by ACHIEVE, one for each of the following audiences: providers, patients, front desk staff in clinic settings.
